**NOTE: This two-session explanatory course teaches how to create a great Advance Care Plan and Lasting Power of Attorney to prepare for a time when you lose mental capacity (e.g. dementia) and cannot care for yourself. The class will not document an Advance Care Plan or Lasting Power of Attorney.


Is having a Will enough? A Will takes effect after death, where they divide our assets to people we chose. But what if we are still alive yet lost our mental capacity, what happens then and how can we plan for it? This course equips participants with the knowledge and skills to choose their care. Discover what makes a great Advance Care Plan and Lasting Power of Attorney. Learn how to make care arrangements for yourself and start the conversation with family and friends about end-of-life planning. Real-life case studies, reflection, and group discussion will be included.

  4. Differences between Advance Care Planning (ACP), Lasting Power of Attorney (LPA), and Advance Medical Directive (AMD)
  5. How do I choose a Nominated Health Spokesperson / Donee?
  6. How do I start a conversation with a loved one?

Pre-requisites
  • 50 years old and above
  • Singaporean or PR
  • Must be able to read, understand and converse in simple English

Your Trainer:

Ms Grace has over 26 years of experience working with the elderly. She currently holds a 1st Class Honours degree in Occupational Therapy and a Master in Gerontology. She is also an Associate Trainer at the Academy of Human Development and the Social Service Institute (SSI), where she received the Adult Educators’ Award in 2014.
